At HCFA we understand that continuous Monitoring and Evaluation (M&E) is vital to ensure that our programs are results-oriented and are making real changes in the lives of the people in our community.
The World Bank has defined 12 components of a functional M&E system for HIV related programs, which are:

- HIV M&E partnerships
- HIV M&E plan
- Costed HIV M&E work plan
- Advocacy, communications and culture for HIV M&E
- Surveys and surveillance
- Routine programme monitoring data
- Supportive supervision and data auditing
- HIV database HIV evaluation, research and learning
- Data analysis, information production and
- Using data for decision making
HCFA uses these components as the framework for our M&E program and uses the United Nations Development Program Handbook on Planning, Monitoring, and Evaluating for Development Results as a guideline in conducting M&E.
